Theatre in the Grove Alcohol Server’s Agreement
Overview
Thank you for volunteering to serve alcohol at TITG. Because we are a nonprofit and alcohol service is regulated by the OLCC, we strictly follow OLCC rules and our house alcohol policy. Each year we ask servers to complete this form again so we maintain current records in case of an OLCC inspection.
How is your personal information used?
Your personal information is used only for TITG’s internal records and to contact you if needed. Access is limited to the small number of board members responsible for maintaining our OLCC compliance records.
How is your OLCC information used?
Your OLCC permit number, dates, and permit image are kept on file for compliance. They may be shown to the OLCC if requested during an inspection. Otherwise, the information is used only for our records and is accessible only to the few board members responsible for maintaining TITG OLCC compliance.
TITG Reimbursement of OLCC Service Permit and Training
If you obtained your alcohol server permit to volunteer at TITG, we will reimburse the permit and training fees up to $60. Please keep your receipts. If you have already received your permit, gather your receipts before continuing because you will be asked to upload them as part of this process. Reimbursement will be issued after you have served twice as an alcohol beverage server.
Documents you need before continuing
Before continuing you will need access to these. Find these before you continue:
- A digital copy of your OLCC permit. It can be an image (JPG, PNG, etc) or a PDF.
- If you want to request reimbursement, then you will need two receipts: one for OLCC training and the other for the OLCC permit.
